Risk Factors Affecting Postherniorrhaphy Urinary Retention

Abstract
Purpose: Postoperative urinary retention (PO-UR) is a common complication after inguinal hernia repair. The purpose of this study was to determine the risk factors for developing PO-UR in patients who underwent inguinal hernia repair.

Methods: Between January 2006 and December 2011, a total of 457 patients underwent hernia repairs for primary unilateral inguinal hernias, at Chungbuk National University Hospital. After 279 patients were excluded for having factors affecting postoperative urinary function, the remaining 179 patients were analyzed. Age, sex, types of anesthesia, types of repairs, duration of operation, presence of co-morbid conditions, other complications, and length of hospital stay, were compared according to PO-UR.

Results: Thirty six out of 179 patients (20.1%) suffered from PO-UR. The average age of patients who suffered from PO-UR was 67.94 ¡¾ 15.64 years, which is significantly higher than that of patients who did not suffer from PO-UR (53.03 ¡¾ 18.26 years) (P£¼0.001). Sex, types of anesthesia, types of repairs, duration of operation, and presence of co-morbid conditions did not seem to affect PO-UR, significantly. Average length of hospital stay was 5.44 ¡¾ 1.88 days, in patients who suffered from PO-UR, which is significantly longer than that of patients who did not (3.27 ¡¾ 1.20 days) (P£¼0.001).

Conclusion: Although elective inguinal hernia repair is a minor operation and serious postoperative complication is very rare, PO-UR is quite a common complication. Advancing age appears to be the main risk factor for PO-UR. In addition, PO-UR significantly prolonged the length of hospital stay. ThereforE, we should prepare for PO-UR after hernia repairs, especially in elderly patients.
